- A completed UCSF referral form
- Patient's demographic information
- Photo of patient's insurance card (front and back)
- Recent lab results
- Previous nephrologist's notes, if patient has seen one
- Preivious kidney biopsy pathology reports, if patient has had one

To refer a patient to UCSF for a native Kidney Biopsy, we require that they be seen first at the Nephrology Faculty Practice. Please follow referral instructions above and indicate on the referral form/fax that you are requesting a kidney biopsy for your patient as well.
